To register for the 2012 Camp Season you must follow these steps: 1) Click on the links below and fill out the required information for each child. Boys Elite Camp at MIT Co-Ed Camp at Solomon Schechter 2) Download and fill out the following forms (linked as attachments at the bottom of the page): a) Waiver and Release Form - REQUIRED! b) Medical Form - REQUIRED! - if there are no medications please list no applications but still complete the form c) If necessary, a form to allow others to pick-up your children from camp, located on the Rules and Regulations page. 3) Obtain a copy of your child's most recent physical exam and immunization records (both must be within a year of the last day of the camp for which you are registering). 4) Send your payment in the form of a check made out to "Coach O's Soccer TEAMS", and the forms listed above to: Coach O’s Soccer TEAMS PO Box 920266 Needham, MA 02492 *Please note: A spot for your child is not reserved until payment is received. Feel free to send the payment, waiver and medical forms right away, and then send the medical records once you obtain them. A minimum of two week's notice must be given if you need to cancel your registration for any reason. |
