Needham Area Week 1 (August 11-15): Still has spots available!

Needham Area Week 2 (August 18-22): Is currently full with a 40+ person waitlist 

Waitlist Information- In a typical year we end up adding 10-20 people from the waitlist for each week. Often times families far down on the waitlist or families who register in July/August get in because others above them opt out as they have made other plans after months of little movement. So if there is a waitlist, don't be discouraged yet! I'll be in touch about your spot in line and more information about the likelihood of getting in. 

Registration Process:

We use JotForm for registration & waivers, payment is done externally, and use dropbox to collect health forms. 

1) Complete the registration through the JotForm link provided in your email or linked above. You will get a confirmation email of your registration. If you see you made a mistake in your registration or need to make a change, please don't register again. You can send an email to jimmyodierna@coachosoccerteams.com to get that fixed.

2) Complete payment of the registration fee amount for your family through either:

• Venmo to @coachOsoccer (preferred method!)  

• Square Payments (click link for payment page)

3) Upload your child(ren)'s health/doctor form (showing the most recent physical and list of immunizations - same one you send into the schools). Use this link to upload the forms for Needham. Use this link to upload the forms for Hopkinton
