Do you have a medical staff?
Yes, there is always at least one registered nurse or trainer on site to care for players who aren’t feeling well or get injured. Our staff of nurses and trainers are highly experienced, working at local public schools and hospitals during the year. We also have an “on-call” doctor to call in case of extreme injuries. Along with the medical staff, Director Jimmy Odierna is fully trained in First Aid and CPR. What medical information do you need? Proof of medical insurance, a copy of a physical exam within the past 12 months and a copy of immunization records are required. Pursuant to state law, no camper will be allowed to participate until all forms are received. In addition, space is not secured in camp until all documents are received. Is there a half day option? Yes for the co-ed day camps, the half day price is $165 (see below in regards to siblings). The half day starts at the same time, but then you will pick up your child at lunch time at 12:30 PM. Half days tend to be good ideas for very young players, although I will say that in the past most campers end up changing their half day sign up to a full day sign up after the first day or two. Your child is tougher than you would think and just want to have fun! Do you have an early drop off period or late pick up period? Yes, we will be offering an early drop off period, from 7:45 to 8:45 AM for an additional $25 per week. We will also have a late pick up period, from 4:15 to 5 PM, also for an additional $25 per week. There will be no trainer/nurse on site during these time periods. Do you have family discounts? Yes, we provide a 10% discount for each additional sibling that you have attending that week. So for full day the cost for a 2nd or 3rd child is only $265 each. For a half day, each additional child is only $150 How do I register? To register you must click on the “Registration” link to the left and sign up online. Once you have submitted your online form, you will receive a verification email with further directions. These directions will ask you to download and complete the waiver and medical forms. You will need to send all of that in with your payment (check made out to “Coach O’s Soccer TEAMS”) within one week of your registration to PO Box 920266, Needham, MA 02492. You will also need to obtain a copy of your child’s medical records (physical within 12 months and immunization records) and send those in as well. What is your refund policy? Campers sent home by the trainer due to an injury or illness will receive a pro-rated refund. If a camper is dismissed from camp, departs camp early, or arrives late for any reason other than injury (doctor’s note required), full payment is required and refunds will not be processed. Cancellations made 2 or more weeks prior to beginning of the session, Coach O will refund all fees except for a $25 service fee. Refunds will not be granted with less than 2 weeks notice except for the event of a medical concern. What is your Federal ID Number? If you need to know our Federal ID number for tax purposes, it is: 26-4543958 Company name is "Coach O's Soccer TEAMS, LLC" |
